Patient Forms
We want to make your visit to Lind Dental Care as easy as possible, even before you walk through the doors. Please take a few minutes to complete your new patient forms prior to your dental appointment in Buffalo Grove, Illinois. You can download and fill out your forms and bring them with you, or you can complete and submit your forms to our office electronically. Financial Options
At Lind Dental Care, Dr. Mark Lind and our team want everyone to have access to dental care in Buffalo Grove, Illinois, and can work with you to find the right solution for your family’s budget.
At our office, we accept most PPO insurance plans but currently cannot accept HMO or DMO plans. With a PPO plan, you will still receive your reimbursement, but we are not “in-network” with any provider. To expedite your appointment, please call our office prior to your visit with your insurance information so that we can confirm your coverage. We can also assist you with filing your claim and provide you with an estimate of the cost of your visit on request. If you do not have dental insurance or if you need to meet the cost of an out-of-pocket expense or co-pay, we also accept:

We can also work with you to create a payment plan.
